PROMO HIGHLIGHTS

1. What is the promo offer?
   Qualified Metrobank credit cardholders can enjoy:
   • PHP 4,000 Cashback for a minimum single-receipt spend of PHP 100,000
   • PHP 12,000 Cashback for a minimum single-receipt spend of PHP 300,000
   Promo is valid only for 0% installment for 12 months plus, Shop Now, Pay Later up to 90 days after.
2. What is the promo period?
   Promo runs from October 15, 2026 to December 15, 2026.
3. How does Shop Now, Pay Later work?
   For qualified Shop Now, Pay Later transactions:
   • Billing starts on the third statement cycle following the transaction date.
   • If the purchase is made before the statement cut-off, billing is typically deferred by 2 months.
   • If the purchase is made after the statement cut-off, billing is typically deferred by 3 months.
4. Are straight purchases included in the promo?
   No. Straight transactions are not qualified for the cashback promo.
5. Are installment terms below 12 months eligible?
   No. Only 12-month installment terms qualify.
6. How much do I need to spend to qualify?
   • Spend at least PHP 100,000 and get PHP 4,000 cashback
   • Spend at least PHP 300,000 and get PHP 12,000 cashback
7. Can I earn cashback more than once during the promo period?
   No. Cashback is capped at one qualified transaction per customer per month.
8. If I have multiple qualified transactions, will I receive cashback for all of them?
   No. Only the first qualified transaction for each month will be rewarded.
   
   If a customer spends PHP 100,000 on November 10 and another PHP 300,000 on November 12, only the PHP 4,000 cashback will be awarded, as only the first qualified installment transaction of the month is eligible for the promo. 
9. When will I receive my cashback?
   The cashback will be automatically credited to your Metrobank credit card account within 30 business days after the end of the promo period.
   
   The cashback credit will appear as: "BELO INSTLMNT CASHBACK"

PROMO MECHANICS

1. The promo is applicable only to World Mastercard, Travel Signature Visa, Femme Signature Visa, Platinum Mastercard, Travel Platinum Visa, Cashback Platinum Visa, Titanium Mastercard, Rewards Plus Visa, Toyota Mastercard, Toyota Platinum Mastercard, M Free Mastercard, PSBank Mastercard, Femme Visa, Vantage Visa & Mastercard, NCCC Card, Puregold Card and M-Lite Mastercard that are in good credit standing.  
   
   The promo is not applicable to Metrobank Corporate Cards, Metrobank Dollar Mastercard, Metrobank Dealer Financing, Metrobank Debit Cards and Metrobank Prepaid Cards. 
2. Qualified Metrobank credit cardholders can avail of any of the following offers at any Belo Medical Group clinic:
   • PHP 4,000 Cashback for a minimum single-receipt spend of PHP 100,000

6. To avail the promo:
   a. Registration is not required. Metrobank credit cardholders may simply use their valid Metrobank credit card upon purchase and meet the minimum spend requirement.
   
   b. The Metrobank credit cardholder must charge the total purchase amount to his/her qualified Metrobank card through the participating clinic’s Metrobank Point-Of-Sale Terminal.
   
   c. To qualify for the cashback promo, the cardholder must have an active account and have no outstanding past due during the period in which the cashback will be rewarded. Otherwise, cashback earned under this promo will be forfeited.
7. Other terms and conditions apply:
   • Installment Terms and Conditions:
     • All 0% Shop Now Pay Later transactions will be billed to the cardholders account starting the third (3rd) statement cycle based on the original Shop Now Pay Later transaction date and every month thereafter until the total amount payable is paid in full.
       To further illustrate:
       • If purchase is made before cut-off date, billing will be deferred for 2 months.
       • if purchase is made after cut-off date, billing will be deferred for 3 months.
     • Full installment amount purchased shall be deducted from the cardholder’s available limit.
     • All installment purchases are subject to the Terms and Conditions governing the issuance and use of Metrobank credit cards and all related provisions of Metrobank credit card’s 0% installment program and only valid if monthly dues are paid in full.
   • Cashback is capped at one-time per customer, per month only.
   • For multiple transactions falling within the promo period, only the first qualified transaction each month will be rewarded. 
   • The cashback earned from the qualified 0% installment or Shop Now, Pay Later transaction will be automatically credited to the cardholder’s account within 30 business days from the end of the promo period with transaction description “BELO INSTLMNT CASHBACK”.
   • Cashback credit earned by a Supplementary cardholder from this promo will be automatically credited to the Principal cardholder’s account.  
   • The promo is non-transferable, non-cumulative, and cannot be exchanged for cash or other products.
   • The promo is not valid in conjunction with other cashback promos of Metrobank.   
   • Splitting of transaction for the purpose of availing multiple cashback is prohibited. Splitting of transaction is defined as an intentional act of dividing a merchant transaction into multiple charge slips to redeem more than one reward incentive. A presumption of splitting of transaction arises when there are 2 or more transactions in one merchant outlet or establishment, and the interval of these transactions were made within 5-10 minutes.
   • The use of the Metrobank credit card in connection with this promo is subject to the Terms and Conditions governing the issuance and use of Metrobank credit cards. In such a dispute, Belo Medical Group shall have no liability.  
   • Metrobank reserves the right to disqualify a previously approved transaction subsequently cancelled or charged back within the promo period. Should a cardholder receive the cashback, but was later on deemed unqualified for not meeting the requirements of the promo due to disputes arising from, but not limited to, erroneous, invalid, fraudulent or unauthorized transactions, the cost of the cashback shall be charged by Metrobank to the cardholder’s credit card account.
   • Any amendment in the terms and conditions is subject to DTI approval.
   • The terms and conditions governing the issuance and use of Metrobank credit cards, reminders and other provisions contained in the card carrier, statement of account, charge slips and other documents or instruments, which are made an integral part hereof by reference, shall likewise be resorted to in instances where they are applicable in this promo.
   • Any dispute concerning the products or services, including pricing, related to the promo shall be settled directly between the cardholder and Belo Medical Group, with the concurrence of the DTI. Metrobank shall be free from any liability in the event of such dispute.
   • In case of dispute in respect of Metrobank only and the cardholder’s availment of the promo, the decision of Metrobank, with the concurrence of the DTI, shall be considered final.
   • By joining or availing of this promo, the cardholder confirms that he/she has read, understood and agreed to the promo mechanics and its terms & conditions.
