HOW TO AVAIL THE 0% INSTALLMENT PROMO PER HEALTH OFFER

  1. All Metrobank Credit Card holders will receive an email, SMS, and Viber message with the promo/campaign URL link.

  2. Copy the link and paste it on your browser.

  3. Choose your preferred product – Global Health Access (GHA), Health Max, or Health care Access (HCA).

  4. Fill out the booking appointment form and submit. An email confirmation will be sent to you along with your FREE Teleconsultation code (Full terms & Conditions in Section III).

  5. Wait for an AXA Financial Executive to contact you.

  6. Settle your premium using your Metrobank Credit Card to avail of 0% installment:

    1. Go to https://www.axa.com.ph/
    2. Go to Customer Service > AXA > Payment
    3. Click the ‘+’ button in 0% installment payment (Exclusive for Metrobank card holders)
    4. Click ‘Yes’ and it will go directly to the One-time payment page
    5. Manually input the policy number and premium amount due
    6. Tick the ‘I’m not a robot’ button
    7. Tick ‘Agree and continue' to enable the ‘Pay Now’ button
    8. Click the ‘Pay Now’ button to proceed
    9. Select Payment Method
      • Over 3 months
      • Over 6 months
    10. Once the ‘Pay Now’ button is clicked, it will be redirected to the Transaction Information page
    11. Transaction is successful once the OTP has been verified

0% INSTALLMENT OFFER OF METROBANK CREDIT CARD

  1. The Promotion is applicable to all Metrobank Peso Visa/Mastercard, Metrobank Vantage Visa/Mastercard, M Mastercard, Titanium Mastercard, Platinum Mastercard, World Mastercard, NCCC Mastercard, PSBank Credit Mastercard, Toyota Mastercard, Rewards Plus Visa, Femme Visa, Femme Signature Visa, Cashback Platinum Visa, and Travel Platinum Visa credit cardholders in good standing. The Promotion is not applicable to ON Internet Mastercard, Metrobank Dollar Mastercard, Metrobank PRIME, Elite, Premier + Debit, Prepaid cards and YAZZ Prepaid Visa.

  2. Avail of 0% installment for 3 and 6 months to pay with your Metrobank Credit Card with a minimum spend requirement of PHP 3,000.

  3. Metrobank Credit Card reserves the right to disqualify a previously approved transaction that was cancelled or charged back within the program period.

  4. Full installment amount under the 0% installment shall be deducted from the cardholders available credit limit.

  5. All installment items and corresponding installment terms are subject to governing the issuance and use of Metrobank Credit Card and all related provisions of the Metrobank Credit Card 0% installment programs.

  6. All 0% installment purchase/s are subject to Metrobank Credit Card approval and are only valid if monthly dues are paid in full.

  7. The terms and conditions governing the issuance of Metrobank Credit Cards, reminders and other provisions contained in the card carrier, statement of account, charge slips and other documents or instruments, which are made an integral part hereof by reference, shall likewise be resorted to in instances where they are applicable in this Promo.

  8. In case of dispute in respect of Metrobank only and the cardholder’s availment of the Promo, the decision of Metrobank with the concurrence of the DTI shall be considered final.

HOW TO AVAIL OF FREE ONE TIME TELECONSULTATION FROM AXA
(Avail FREE Teleconsult when customer books an appointment with an AXA Bancassurance Financial Executive)

  1. The Free Teleconsultation shall be extended to customers who booked an appointment with a Financial Executive from July 12, 2022 to December 31, 2023 (“promo period”).

  2. The promo is open to prospective and existing Metrobank clients.

  3. To qualify, Metrobank customers should book an appointment with an AXA Bancassurance Financial Executive through the form link of their preferred health plan here: Get an AXA health insurance plan at 0% installment

  4. For every successful appointment booked, customers will be entitled to a one-time free teleconsultation with any MyPocketDoctor-accredited doctor, valid until December 31, 2023.

  5. To book an appointment with an AXA Bancassurance Financial Executive, customer must:

    1. Provide the following information:
      1. First Name
      2. Last Name
      3. Email address
      4. Mobile Number
      5. Preferred Metrobank Branch of Account
    2. Provide consent to:
      1. Use their personal information to evaluate and assess the request to book an appointment with an AXA Bancassurance Financial Executive to discuss Health and Protection Financial Solutions.
      2. Use their personal information (name, email, mobile number) to avail of the FREE one-time teleconsultation.
  6. Qualified customers will receive one (1) email from AXA on the same day they book an appointment. Email contains their unique code and instructions on how to redeem their free one-time teleconsultation, and confirmation on appointment booking form

  7. To redeem the free one-time teleconsultation, customer must:

    1. Download the EMMA by AXA PH mobile application via GooglePlay or App Store
    2. Open the app and go to the Health Hub section. Click LEARN MORE and choose Teleconsultation
    3. Select ADD COUPON CODE and input the unique code found in the email sent by AXA Philippines
    4. To proceed, click REQUEST TELECONSULTATION and send the pre-populated SMS.
    5. Wait for a call from MyPocketDoctor.

Terms & conditions of the free one-time consultation from AXA

  1. The unique code for the free one-time teleconsultation from AXA can only be used once.

  2. The code is transferrable. Once availed, customer may not request for another code.

  3. All booked but unavailed teleconsultation from the promo shall be forfeited after the availment period.

  4. The offer cannot be used in conjunction with other discounts, promotions, discounted items, and fixed price items, unless specified.

  5. Global Health Access, Health Max and Health Care Access, and other financial products of AXA are neither guaranteed by PDIC nor by Metrobank. Any concerns, issues or complaints related to this application or your purchase will be handled by AXA Philippines. Metrobank shall not be liable for any claims, losses, damages, and/or expenses arising from this transaction.

  6. Medical opinions and recommendations from the Emma by AXA PH app are rendered by an independent provider and does not bind Metrobank or AXA Philippines in any way. They are not given nor intended as medical opinions/ recommendations from Metrobank or AXA Philippines
